The Department of Family Medicine is undergoing a survey. As part of this process, the survey committee is looking for feedback from the members of the department and from the broader medical school and university communities on its perceptions and experiences working within/with the department.

Your identity cannot be linked to your responses and only aggregated results will appear in any reports or communications regarding the survey results.
